T1204: Government Service Contract Payments
The T1204 slip is issued by federal departments, agencies, and Crown corporations for service contract payments made to individuals, partnerships, small businesses, or corporations. A T1204 slip might be issued to you for payments that were more than $500 in the year (excluding GST/HST or provincial/territorial tax).
Note: The payer is only required to submit the T1204 slip to the Canada Revenue Agency (CRA) but might send you a copy. Be sure to keep a record of all the payments you received from the government for services and/or goods you provided.
If you received the T1204 slip, you will need to complete the T2125: Statement of Business or Professional Activities form to determine the amount of taxable income to report on your tax return.

Follow these steps in H&R Block’s 2024 tax software:

On the left navigation menu, under the Credits & deductions tab, click Tax Topics.

Select the Self-employment income & expenses checkbox.

At the bottom of the page, click Add selected topics to my return.

Click the Employment tab on the left navigation menu.
- Under the BUSINESS AND SELF-EMPLOYMENT INCOME heading of the Employment and self-employment page, select the checkbox labelled Statement of business or professional activities (T2125), and click Continue.
- Enter the amount reported in box 82 or box 84 of your T1204 slip as revenue in the Income section on the T2125 page.
